We support one another.POSITION SUMMARY: The Community Support Specialist will provide day-to-day management of CF Peer Connect, with special focus on mentor recruitment, screening, registration, support, and engagement.
ESSENTIAL DUTIES AND RESPONSIBILITIES:Facilitate the day-to-day operations of the peer mentoring program;Assist individuals providing mentorship through the registration process including:Process new applications for CF Peer Connect mentors;Recruit mentors in needed areas;Conduct phone screening interviews with potential mentors;Ensure mentor training and support;Assist Sr. Community Support Specialist with making personalized “matches.”Maintain follow-up communication and match support with active participants;Provide counseling, guidance and follow-up to mentors throughout the program in line with program guidelines;Engage in continuous quality improvement of program processes;Assist efforts to develop program-specific communication materials;Develop and deliver presentations to internal and external audiences (CFF chapter staff and care center teams) to increase awareness and understanding of peer connection programs;Organize event activities at foundation-wide events as needed, including the Volunteer Leadership Conference and the North American Cystic Fibrosis Conference;Other duties as required by the Sr. Manager of Community Support;Occasional domestic travel may be required.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:Bachelor’s degree required; master’s degree preferred. Health-related field strongly preferred.Minimum of four years of relevant experience with community engagement, social work, or patient care.Excellent verbal and written communication skills. Counseling and/or patient care experience strongly preferred.Basic understanding of psychosocial aspects surrounding chronic medical conditions such as cystic fibrosis.Strong analytical skills.Well-organized with strong attention to detail.Team player with a positive attitude and strong work ethic.Knowledge of HIPAA and other patient-centered compliance issues preferred, but not required.General knowledge and comfort with new technology platforms and databases.Proficiency using Microsoft Office Suite. Experience with complex Power Point presentations and Excel reporting preferred.This is a part-time, permanent position. Remote work is possible.Some hours may take place outside normal business hours (5-9 pm ET) in order to communicate with program participants in Pacific and Mountain time zones.
